Six years ago (2000), a military helicopter belonging to an American Special Forces unit was downed during the Penamstan Civil War. Taking part in the same operation, the Mad Dog Squad ignored orders to stand by and went in to search for survivors. Little did they know the unit had already been wiped out. The Mad Dog Squad, including their leader Jason, barely made it back in one piece.

However, Jason witnessed the “dead” members of the Special Forces unit unnaturally lurch their way from the crash site... Present day (2006), top-secret files on the Presidential Network at the White House were compromised. Agent Leon S. Kennedy has been called in to investigate the incident. Working in a team of four that includes Penamstan Civil War hero Jason, the lights suddenly go out and the agents encounter unidentified zombies within the White House. SWAT assists them in neutralizing the situation.

It is discovered that the top-secret files were connected to a biology lab in Shanghai after the attack. Leon and the agents decide to pursue the lead and make for Shanghai in a cutting-edge submarine. A horde of rodent B.O.W’s attack them en route, plunging the group into desperate straits.

Meanwhile, TerraSave staff member Claire Redfield comes across a mysterious image drawn by a mute child in a Penamstanian refugee camp while working there. Seeming to depict a person suffering from a viral infection, Claire begins an investigation of her own. Her quarry leads her to uncover gruesome experiments carried out at the height of the war. Leon and Claire close in on the unthinkable reality of incidents that took place during the Penamstan Civil War. Worse, a manifesting terror that threatens the peace of the world is brought to light...
